Filler dissolving (hyaluronidase) in Naples, FL

Hyaluronidase is an enzyme that breaks down hyaluronic acid filler within hours. It is used to correct over-filled lips, migration, lumps or a bluish tint under the eyes, or simply to start again. Dr. Irina Toma, DDS, a Florida-licensed dentist (FL Lic. DN25459), dissolves her own work and work done elsewhere, when the filler is hyaluronic acid.

What filler dissolving is

Hyaluronidase (brand names Hylenex, Vitrase) is an enzyme that digests hyaluronic acid, the material in Juvéderm, Restylane, RHA and Revanesse fillers. Injected into the area, it breaks the gel down within hours; swelling settles over a day or two and the result is visible at about 48 hours.

People search for this as "filler dissolver", "lip filler removal", "dissolving lip filler", "hyaluronidase injection" or "filler reversal". It is the same treatment. It works only on hyaluronic acid fillers; Sculptra, Radiesse, silicone and fat cannot be dissolved.

Dr. Toma uses it for three reasons: to correct a problem (migration above the lip line, lumps, a bluish Tyndall tint under the eyes), to reset before planning fresh work, and urgently for a suspected vascular occlusion, which is why hyaluronidase is kept available whenever hyaluronic acid filler is placed.

Off-labelHyaluronidase (Hylenex, Vitrase) is FDA-approved as an adjuvant to increase the dispersion and absorption of other injected drugs and for subcutaneous fluid administration. Dissolving hyaluronic acid filler is a well-established off-label use, discussed and consented to before treatment. Verified Oct 2026.

What can and cannot be dissolved

If you do not know which filler you had, a small test area is treated first.

Filler typeExamplesCan it be dissolved?
Hyaluronic acid (HA)Juvéderm, Restylane, RHA, Revanesse, BeloteroYes, usually in 1 to 2 sessions
Calcium hydroxylapatiteRadiesseNo; it is absorbed gradually over many months
Poly-L-lactic acidSculptraNo; the collagen it stimulates is your own tissue
Permanent fillersSilicone, PMMA (Bellafill)No; a surgical opinion is needed
Fat or allograft matrixFat transferNo

Denser, more cross-linked HA (such as Voluma) can need more enzyme or a second session than softer lip gels.

Partial dissolving

Small, diluted doses can soften a specific lump or a migrated edge while keeping the rest of the result. Full reversal uses a higher dose across the area.

Allergy

True allergy to hyaluronidase is rare but possible. A forearm patch test is offered about 20 minutes before treatment, and is advised if you have a bee or wasp venom allergy.

Common reasons

ProblemTypical approach
Lip filler migration (above the lip border)Targeted dissolve; refill after about 2 weeks if wanted
Over-filled or uneven lipsFull or partial dissolve, 1 to 2 sessions
Under-eye puffiness or bluish tint (Tyndall effect)Low dose, often one session
Lumps or nodulesSmall diluted dose into the lump
Old filler before new planningFull reset, then a plan at the two-week review
Suspected vascular occlusion (emergency)High-dose hyaluronidase at once, repeated as needed, with emergency care

Dissolving is quoted separately from any new filler.

Who it is for

A good fit if
  • Lips that look migrated, heavy or "ducky"
  • Lumps, asymmetry or a bluish tint from a previous filler
  • You had filler elsewhere and want a clean start
  • You want less, not more
Not for you if
  • Known allergy to hyaluronidase (patch test first if you have a bee or wasp venom allergy)
  • Active infection at the site
  • Pregnant or breastfeeding
  • Non-HA fillers (Radiesse, Sculptra, silicone)
What it can't do
  • Dissolve Radiesse, Sculptra, permanent fillers or fat
  • Remove your own natural hyaluronic acid permanently; the skin replaces it within days
  • Guarantee a one-session result for dense, old product

How much does filler dissolving cost?

Priced per session, with a lower rate for a small targeted area, stated as a minimum fee with the dates it is in effect. When a planned adjustment of Dr. Toma's own work is part of your written plan, it is covered by that plan. Everything is in writing before treatment.

Prices are confirmed at consultation and stated with their effective dates. Any offer carries its start and end dates. How pricing works.

What happens, step by step

  1. Consultation
    Identify the filler

What was injected, where and when. If unknown, a small test area is treated first.

  2. Day 0
    Patch test and treatment

Optional forearm patch test, about 20 minutes. Then diluted hyaluronidase is injected into the filler and massaged. About 30 minutes.

  3. Hours 1 to 24
    Breakdown

The gel softens within hours. Expect swelling; the area can look worse before it looks better.

  4. Day 2 to 3
    Result visible

Swelling settles and the result shows. Photos compared.

  5. Week 2
    Review and refill

A second session if dense product remains, or new filler once the area has settled, if you want it.

Risks and side effects

Common: swelling for one to three days, bruising, tenderness. Uncommon: temporary loss of some natural volume, because the enzyme also breaks down your own hyaluronic acid; it rebuilds within days. Rare: allergic reaction, including anaphylaxis, which is why a patch test is offered and emergency medication is available.

Full informed consent is reviewed before treatment.

Questions about filler dissolving

Hyaluronidase, an enzyme that breaks down hyaluronic acid filler. It is searched as filler dissolver, lip filler removal or filler reversal; it is the same treatment.

The enzyme works within hours; swelling settles in one to three days and the result is clear at about 48 hours. Dense or old filler may need a second session about two weeks later.

It also breaks down some of your own hyaluronic acid, so lips can look slightly deflated for a few days, then recover.

Yes, if it is hyaluronic acid. Bring whatever you know about the product. If it is not hyaluronic acid, we will tell you honestly that it cannot be dissolved.

About two weeks, once swelling has settled and the true baseline can be seen.

A brief sting. Lidocaine is mixed in and ice is applied. Most people describe it as easier than the original filler.
